Breast Cancer Treatment Booklet The Breast Cancer Informed Consent Law (Michigan Public Act 368 of 1978, as amended by Public Act 195 of 1986 and Public Act 15 of 1989) requires Michigan physicians responsible for administering treatment to breast cancer patients to inform them about alternative methods of cancer treatment, including surgical, radiological, or chemotherapeutic treatments, or any other generally accepted medical treatment. This explanation should include discussing advantages, disadvantages and risks associated with each method of treatment and documented in the patient's medical record. In a collaborative effort, the Michigan Department of Community Health, the Michigan State Medical Society, the Michigan Osteopathic Association, the Michigan State Medical Society Foundation, Blue Cross Blue Shield of Michigan, and the American Physicians Assurance Corporation created this 50-page booklet to help physicians, patients and families deal more effectively with dying and death. Caring Choices: A Guide to End-of-Life Decisions and Care Published in 2002 by the Michigan Partnership for the Advancement of End-of-Life Care, this guide is designed to help people to think about what they want in end-of-life care, ask for what they need, and understand some of the steps they need to take to make sure they receive the care they desire. It also includes the results of the Commission's survey on the availability and cost of public and private insurance coverage for hospice services, pain management, and palliative care.

The MCC Prostate Cancer Action Committee operates this site to help inform early prostate cancer patients. The site includes an easy-to-navigate Web version of the MCC booklet Making the Choice: Deciding What to Do About Early Stage Prostate Cancer, as well as an interactive tool that enables patients to determine five-year survival rates using their prostate specific antigen (PSA) test results, tumor state, and grade.
Web site visitors can download an Adobe Acrobat PDF English- , Spanish- , or Arabic-language version of the Making the Choice booklet, or they can order it in a color-printed format or as an audio set containing one CD and one cassette. A link to an Adobe Acrobat PDF English-language version of the booklet as adapted by the South Puget Intertribal Planning Agency in Shelton, WA, for use by members of five Northwest Tribes (Chehalis, Nisqually, Shoalwater Bay, Skokomish and Squaxin Island) is also included.
