On the Voluntary Contributions Schedule (Form 4642) associated with your 2009 Michigan Individual Income Tax Return (Form MI-1040), you’ll find check boxes to donate $5, $10, or more to cancer support and research in Michigan.

  • Print off the "Use Your Tax Return to Fight Cancer" poster (available as an Adobe Acrobat PDF file)* and display it at:

    • workplaces;

    • local health agencies, doctor's offices, and clinics;

    • libraries, post offices, churches, and municipal buildings, such as city halls;

    • special sporting events and concerts;

    • health fairs; and

    • retail shops, shopping malls, restaurants, and beauty shops.

  • Print off and distribute the "Use Your Tax Return to Fight Cancer" flyer (available as an Adobe Acrobat PDF file)*

  • Encourage your local station to use the radio public service announcement (PSA) promoting the Michigan Tax Check-Offs to Fight Cancer. (Note: This MP3 audio file is 2.29 MB)
